Attention Hospital, FQHC, and Rural Health providers billing crossover claims on the Institutional claim form (UB-04, 837I):
To ensure correct reimbursement when adjusting or resurrecting Institutional Medicare crossover claims in the Direct Data Entry (DDE) Portal, always verify that the Medicare coinsurance/deductible information is listed correctly in the claim level expanded section under the question “Is this a Medicare crossover claim?” Failure to verify this information could cause the claim to deny or be paid at Zero dollars.
